One of these key reliability work processes is Root Cause Analysis or RCA.  RCA is a series of steps taken to determine the underlying causes of plant problems.  These problems range from costly equipment failures to safety and environmental incidents.  The key difference between a proactive plant and a reactive plant is how they manage equipment failures.  Reactive plants simply identify that a problems exists and do what ever it takes to get the problem resolved so plant operations can continue.  Proactive plants find out how the event occurred in the first place and put in corrective actions to ensure that the problem does not reoccur.
